All that we ask of you is to fill in the form below. After we received the information, we will then begin to schedule the night we are sure you'll never forget!
Please understand that it is very important for everyone that is signed up to attend and be on time. We ask that you arrive at 5:15. If you are unable to be on time, please understand that there may be shortened service times to accommodate all guests.
If you are interested in having a light dinner, we will provide this for you with an extra charge of $5.00 per person. We will serve chicken salad sandwiches, chips, fruit, and a dessert. If you have any questions or special requests, please let us know, so that we can accommodate your needs. Thank you.
